JOB DESCRIPTION:

Set up in-home therapy care for patients throughout California by matching the needs of the patient with the appropriate clinician. Communicate with other healthcare professionals (clinicians, home health agencies, physicians) to ensure that our patients are receiving the best care possible.

Coordinate and manage the care of hundreds of patients throughout California. Ensure that clinical care is being completed according to set Medicare standards, and, when those standards are not met, resolving the subsequent issues in patient care. Review clinical documentation and ensure its timely and complete submission.

A well qualified candidate would:

- Have excellent interpersonal, communication, and customer service skills- verbal and written

- Typing speed about 45- 50 words-per-minute

- Be detail-oriented

- Have impeccable follow-through

- Work well on a team

- Be adaptable and willing to learn

- Be resourceful and solution oriented

- Be fun and outgoing
