Advanced Home Health & Hospice has a part time or per diem need for Masters level social worker. Ideally, candidate will have at least 6-12 months experience in Medical Social Work role. Our preferred applicant must have a Masters Degree from a school of social work accredited by the Council on Social Work Education, and have at least 1 year of social work experience in a healthcare setting. We are looking for a candidate to support the Yuba City / Marysville area on a per diem basis.
As a Social Worker for Home Health and Hospice, MSW will visit patients, family members and caregivers in the home or residential facility and assess their needs in their residential setting. May provide assistance with resources and for hospice, palliative or home health care needs. Will work with clinical team members; RN, LVN, PT, PTA, OT, Chaplain and CNA / CHHA.
