Job Description: Provide one on one job coaching services in the community through two programs; job coaching and job development. The job coach ensures all contact notes, Colorado Division of Vocational Rehabilitation progress reports and any other program related documents are submitted in a timely manner. Training is Provided.
Job Duties:
Follow Client's Service Plan and DVR authorizations
Attend meet and greets for supported employment and job coaching
Ongoing communication with supervisor, team members and family members
Attendance to team meetings as needed
Provide accurate documentation of job coaching and job development contact notes and DVR progress reports
Perform the duties of job coaching (client's work site) and job development (skills building via job interviewing skills, helping fill out job applications and job searches)
Ensure the safety and well-being of all clients
Numerous miscellaneous duties that may come up as a result of anything stated above
Observe confidentiality of all PPCH clients and operations
Must be able to show competency in all PPCH meetings and trainings
Ability to understand the DVR billing process
Other duties and responsibilities as assigned, within ability and responsibility level of the job

Qualifications:
At least one year of experience working with adults with developmental disabilities
Good organizational skills and communication skills
Must be team oriented
Understanding of the DVR process is a plus
Experience working with DVR or with assisting adults with developmental disabilities in securing employment
Outstanding attendance and ability to self -direct
Valid Colorado Driver's License
Good driving record that meets the requirements of agencies insurance carrier
Able to pass criminal background checks
Able to document right to work in the U.S.
Professional attitude and teamwork mentality
Daily significant contact with agency personnel and its employees or contractors as well as outside governing agencies, benefit carriers, and with individuals with Developmental Disabilities, community members and professionals within the community.
