CarpeVITA HomeCare of Delaware is in need of Registered Nurses to join our Case Management Team in New Castle County, Kent County and Southeastern Pennsylvania.
Job Summary: Provides direct supervision of home care personnel; coordinates care
services by preforming an initial & follow up Assessments and creating appropriate Care Plans.
DUTIES AND RESPONSIBILITIES:
* Directs home healthcare by writing a Care Plan. Reviews all plans for accuracy, appropriateness, and completeness.
* Maintains accurate records of all services rendered to care recipients.
* Evaluates and regularly reevaluates the home care needs of each care recipient.
* Supervises and trains other home care personnel. Provides orientation to new personnel.
* Performs Care Service visits on each Home Health Care recipient every 60 days.
* Performs initial admission assessment and evaluation of the Agency's care recipients.
* Oversees the performance of home care in an environment that optimizes care recipient safety and reduces the likelihood of medical/health care errors.
* Reports to the family the care recipients' response to the program and also to changes in their physical or emotional condition.
Professional Requirements:
* Adheres to dress code, appearance is neat and clean.
* Completes annual educational requirements.
* Maintains Qualifications.
* Maintains care recipient confidentiality at all times.
* Represents the organization in a positive and professional manner in the community.
* Complies will all organizational policies regarding ethical business practices.
* Ensures compliance with policies and procedures regarding operations of the Agency.
* Communicates the mission, ethics, and goals of the organization. Supports and carries out
the Agency's policies.
* Drug, Child and Abuse Registry and Criminal Background Checks
* PPD Annually & CPR Certified
Professional Qualifications:
* Registered Nurse with a Bachelors Degree in Nursing.
* Current license from the Delaware State Board of Registered Nursing.
