Our Aftercare Specialist will facilitate mental health testing at the reception to new inmates in order to determine the psych level of the inmates. Then, at 180 days prior to the inmate's release, our Re-Aftercare Specialist will begin working with the inmate to provide guidance on where he will go after release. ÂThis person will also assist the inmate in enrolling in social security/disability benefits as needed.
Additional job duties include:
Maintaining compliance with all facility and MHM policies, federal and state laws, regulations and guidelines including HIPAA
Assess the offenders' post release needs and the services necessary to meet those needs
Act as a liaison between Correctional healthcare providers and community agencies and healthcare providers
Contact family and identify other community support systems
Participate in MHM QI program and attend safety trainings

Qualifications include:
Bachelor's Degree in Psychology, social work, counseling or related field from an accredited educational institution
Minimum of 2 years' experience in release planning preferred
MH testing experience preferred, but not required
Prior experience in corrections, criminal justice or forensics preferred
Current CPR certification
