SHIFT: Days (rotating weekends)The RN Case Manager provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care, and is discharged to the least restrictive environment.

Qualifications
Licensure/Education:
Current State of Florida RN licensure
Completion of an RN Diploma program or related healthcare program
National Case Management Certification strongly desire
Qualifications:
Minimum of three (3) years recent experience in an acute-care hospital setting required
Case Management experience is preferred
