The B Good Group is seeking a contracted Physical Therapist (PT) to provide care to patients throughout Manatee County.
Education/Certification/Experience for a Physical Therapist in Home care:
PT license required.
Home Health experience required
Essential Functions/Responsibilities for a Physical Therapist in Home care:
Develops and revises a Physical Therapy Treatment Plan based on client assessment and information provided through collaboration with the physician and other Agency personnel.
Ongoing assessments to determine level of functioning including OASIS assessments at appropriate time points.
Establishes long and short term goals.
Secures orders for physical therapy every 60 days from the patient's physician.
Applies appropriate physical therapy procedures to achieve identified goals.
Interprets to the family the patient's total Physical Therapy Program including treatment, goals, frequency, duration of visits, and necessity for ongoing assessment.
Provides Physical therapy under the Medicare Program within the scope of covered services as outlined in the Medicare Manual.
