POSITION: Per Diem MANAGING RN / CASE MANAGER

POSITION TYPE: Manager

SUPERVISOR: Director of Nursing

PAY RATE: Competitive

HOURS: 10-20 Per Week

POSITION REQUIREMENTS:

The part time RN / CASE MANAGER is responsible for the service provided to the clients of Always Best Care Senior Services, (the Agency), and for supervising the delivery of services, according to the philosophy, standards and all ACHC Accredited Policies and Procedures of the Agency. The RN / CASE MANAGER will ensure that all assigned direct care staff provides safe, appropriate, prompt, courteous and professional service to clients according to the client's Plan of Service. The RN / CASE MANAGER will supervise the services provided by all assigned Homemakers/Companions, Home Health Aides, Certified Nursing Assistants and LPNs. Office location is in Hobe Sound. Looking for someone located in Palm Beach County. ie. West Palm Beach, North Palm Beach, Palm Beach Gardens preferable.

o Minimum 5 years experience as a RN.

o Minimum 2 years experience with in-home care or home health.

ESSENTIAL FUNCTIONS:

o Conduct assessments of assigned clients as per Agency AHCA policy.

o Create Plan of Service for clients admitted to the Agency.

o Provide case management of specifically assigned clients as per Agency policy.

o As additional employee positions are created, or as vacancies occur, assist the Administrator and Operations Manager in recruiting personnel, as needed.

o Participate on the Quality Assurance committee and insure the quality of services is maintained.

o Comply with all federal and state rules, regulations, and laws, with respect to home health care service.

o Comply with infection control regulations, aseptic techniques, and OSHA Rules.

o Comply with company ACHC policies and procedures.

o Other duties and special projects as assigned.

SUPERVISORY RESPONSIBILITIES:

o Ensure that the all legal standards of practice are maintained by all employees providing service on behalf of the Agency.

o Supervise, monitor and evaluate assigned employees who provide direct service to Agency clients.

o Conduct competency evaluations of potential direct client service personnel prior to the Administrator's final approval to hire and conduct performance evaluations of direct client service employees as assigned by the Director or Nursing.

o Participate in the selection, training, discipline, and termination of client service employees.

o Conduct and supervise Case Management Functions.

o Always retains the full responsibility for the care given to each client under his/her supervision and for making supervisory visits to the client's home.

o Train and conduct the orientation of all direct care personnel as per relevant policies and procedures.

o Be available for supervision of direct service employees as assigned by the Director or Nursing.

QUALIFICATION, KNOWLEDGE AND SKILLS:

o Minimum of two years of home care experience as a professional nurse is required, with one year of experience in a supervisory position preferred.

o Clear a Level 2 Criminal Background screening through AHCA, the OIG Exclusion List and a National Sex Offender Registry check.

o Be licensed, in good standing, as a Registered Nurse in the State of Florida.

o Graduate of a school of professional nursing is required; Baccalaureate degree in nursing preferred.

o Superior organizational, English language communication (verbal and written) and customer service skills.

o Ability to interact professionally with clients, physicians, vendors and Agency employees.

o Ability to work in a demanding and fast-paced environment.

FISCAL RESPONSIBILITIES: None

EXTENT OF PUBLIC CONTACT:

o Considerable personal and telephonic contact with clients, caregivers and client's family members.

o Some telephonic contact with physicians.

PHYSICAL DEMANDS:

o Driving throughout the Agency Geographic Service Area; significant telephonic contact in addition to individual meetings with clients, clients' families and caregivers; lifting boxes of supplies weighing no more than 10 pounds.

WORK ENVIRONMENT:

o Office environment and in clients' homes.
