Quality Management Coordinator for Continuum of Care conducts quality outcome reviews as it relates to the SNF/ICF, care group three and four population in the hospital and outside facilities. Coordinates and facilitates quality related task forces of key physician groups, managers, supervisors, and frontline staff involved with SNF/ICF and care groups three and four patients. Evaluates our SNF/ICF and care group three and four clinical information, recommends interventions, and follows through with execution of the proposed interventions in partnership with physician groups, managers, supervisors and frontline staff. Participates in quality information initiatives related the SNF/ICF and care group three and four members. Develops and manages effective use of process improvement to ensure systems improvements. Assists in establishment of work standards and procedures. May assume role of Director when necessary.Essential Responsibilities:

Assists in defining goals, scope, assumptions, and data needs for the SNF/ICF patients and care group three and four. Assists with coordination with outside facilitates teams of key physicians, managers and supervisors and frontline staff as directed.

Assess and evaluates plans of care for care group three and four on a timely basis, monitoring completeness and appropriateness.

Monitors the progress of care group three and four for emerging complex placement needs.

Collaborates with PCC and Social Services, Rehabilitation Services, SWITCH Navigators and physician to appropriately amend plan of care.

Performs. concurrent and retrospective reviews and monitors level of quality of care at outside facilities.

Coordinates discharges to outside facilities for patients with complex care plans with physician and appropriate disciplines.

Collects, compiles, and analyzes utilization and quality data related to SNF/ICF, care group three and four patients. Evaluates quality, service and utilization outcomes and recommends interventions. Follows through with intervention in partnership with physician groups, managers, supervisors and frontline staff.

Assists in monitoring and recommending changes in methods and procedures, and establishing work priorities to assure timely flow and efficiency, as appropriate. Acquires and interprets SNF/ICF and care group three and four data for decision making.

Participates in quality, SNF/ICF, and care group three and four related committees, quality improvement projects and utilization reviews.

May perform patient care to the extent necessary to maintain clinical expertise, competency and licensing necessary to fulfill job responsibilities and to direct the provision of care in the community and institutional setting.

Incorporates the KP Nursing Vision, Model and Values throughout their Nursing Practice.

Basic Qualifications:Experience

Minimum one (1) year of quality management, quality assurance, public policy, governmental affairs or related experience.

Education

Bachelor's degree in nursing OR four (4) years of experience in a directly related field.

License, Certification, Registration

Valid Hawaii RN license (must meet education requirement(s) for Hawaii State licensure) upon date of hire.

Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.

Additional Requirements:

Demonstrated knowledge of and skill in interpersonal relations, oral communication, written communication, influence/negotiation, organization, planning, analysis, and quality management.

Demonstrated knowledge of and skill in word processing, spreadsheet, and database.

Preferred Qualifications:

Master's degree in nursing, allied health, economics, business, or related field.
