Duties: Provides skilled Physical Therapy services, on an intermittent basis, to patients, in their homes, under direction from the Registered Physical Therapist. Flexible scheduling. Responsibilities: Perform delegated patient care tasks under the supervision of a Registered Physical Therapist (PT). Assist with the evaluation and development of a rehabilitative plan of care. Participate in the assessment and periodic re-evaluation of patients as required. Documentation and Reporting: Prepare and maintain clinical and daily progress notes for each patient, ensuring all notes are reviewed and submitted according to agency policies. Prepare summary reports as directed by the supervising PT. Record activities, findings, and patient reactions to treatment, and report any changes to the PT and the physician. Therapeutic Interventions: Under the PT\'s direction, assist patients with therapeutic exercises, strengthening activities, and the use of self-help devices. Provide guidance on exercise programs, including patient and family education. Monitoring and Reporting: Identify and report any concerns regarding medication therapy, adverse drug reactions, allergies, or contraindicated medications to the PT. Team Collaboration and Education: Work closely with the PT to instruct other health team personnel, including Home Health Aides, and family members about the physical therapy program. Assist with the training of other staff and families to ensure effective care at home. Consultation and Communication: Participate in patient home visits and provide written feedback to the PT regarding physical findings. Engage in case conferences and staff meetings to discuss patient progress and team collaboration. Discharge and Planning: Take on any other responsibilities as directed by the PT, Registered Nurse Case Management, or Supervisor. Skills and Qualities: Strong communication skills for reporting patient progress and coordinating with the healthcare team. Knowledge of physical therapy principles, therapeutic exercises, and self-help device usage. Attention to detail for accurate documentation and patient care planning. Ability to work collaboratively in a multidisciplinary team environment. Dedication to ongoing professional development. This role requires a PTA to not only assist in therapy but also play an active role in patient education, coordination of care, and communication with the entire healthcare team to ensure that the patient is receiving the best possible care in line with their therapy goals. As a Home Care Supervisor, you will play a vital role in the planning, coordination, leading, controlling, and evaluation of home care services.
