HELPING HANDS HAWAII IS SEEKING FULL TIME CASE MANAGERS:

The Case Manager is a member of a multi-disciplinary team responsible for ensuring members are provided with community based case management. Case Management programs are funded by the State of Hawaii Department of Health & Department of Human Services in collaboration with various funders. Case Management services are voluntary services that enhance the consumer's strengths in managing their mental illness, symptoms, and psychiatric history. The program operates under the direction of Helping Hands Hawaii (HHH), Medical Director, Qualified Mental Health Professional (QMHP) and Program Director, as well as the Clinical Team Leader; to ensure adequate support and safe, structured, and therapeutic environment in which consumers receive case management services. The Case Manager is to comply with the company code of ethics, confidentiality practices, HIPAA, safety and all other applicable policies and procedures. The Case Manager reports directly to the Clinical Team Leader and works collaboratively on the clinical team.

Qualifications:

*Bachelor's degree, with a minimum of twelve (12) semester credit hours in courses such as counseling, criminal justice, human services, psychology, social work, social welfare, sociology, or other behavioral science. Master's degree preferred.

*An exception may be made for the above-mentioned educational requirements, on an individual case-by-case basis, if a candidate meets the following combination of required minimum education & credentials:

RN license. May have a Bachelor's or Associate's degree in any field.

1-1/2 years of specialized experience.

*Must have strong commitment to the right and ability of each person with severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.

*Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for client rights and personal preferences in treatment are essential.

*Must have a smartphone.

*Strong punctuality, timeliness of documentation.

*Strong clinical skills working with severe and persistently mentally ill.

*Must have own vehicle, Valid Hawaii Driver's license, clear traffic abstract and willingness to use properly insured vehicle. clear pre-employment reference check, annual TB Clearance, CPR, First Aid and CPI certification, clear background check.
