Responsible to evaluate and meet the on- call after hour needs of hospice patients of assigned office. Responds in courteous, timely and professional manner
Is knowledgeable of and safely provides pharmacological and non- pharmacological interventions to promote comfort and symptom management within scope of practice, competencies, and as directed by patient's physician and plan of care
Communicates and updates RN Case manager and other IDT team members as indicated, supporting holistic care of hospice patients and their families
Provides thorough and accurate documentation of all after- hours cares and services provided in clinical documentation using SOAP format. Syncs within HNH 12-hour documentation policy
Follows the individualized Plan of care in a consistent and appropriate manner. Suggests any changes needed to case manager based on after hour services provided
Possession of a current RN/LPN License in good standing in respective state
At least 2 years of home care experience preferred
Strong written, oral and technical skills including but not limited to use of computer, cell phone
Demonstrated ability to take direction and respond to needs of patients and their families in varied settings
Ability to handle multiple requests for care prioritizing and requesting assistance and support as needed
Must be a licensed driver with an automobile that is insured in accordance with state and organizational requirements
Criminal history clearance
Ability to handle emergency and end of life situations within scope of practice and competencies
Physical Requirements Include:
Successful completion of physical exam and ability to lift a minimum of 75 pounds on a consistent basis
