Temporary Health Care Coordination Representative Counselor: Champaign, IL
The Care Coordination dept. Carle Foundation Hospital is comprised of care coordination representatives, social workers/social service support specialists and RN care coordinators. The multidisciplinary team provides primarily telephonic support to members to remove any barriers for members reaching optimal health. The Care Coordination team provides varying levels of support to ensure the right care at the right time. Start: ASAP M-F 8a-5p. Pay rate is $15/hr.
Requirements
Two (2) years in a business setting with strong emphasis on customer service and handling confidential matters. Pref Exp: n/a Dress Code: Business casual.
Primary functions would include activities such as conducting telephonic outreach to members to engage them into the care coordination program; take inbound calls from members seeking to enroll; assist with mailing. Length of Assignment: 2/27/19 -5/25/19. Filling in FMLA leave.
