Looking for a Licensed Social Worker / Certified Care Managers / RN to meet with senior citizens in their homes to help assist with Care Coordination & Care Management. No hands on care. This is to conduct assessments only. Meet with senior citizens once a week or Biweekly ( It depends on the program) in their homes for about an hour per visit. Pay Rate: $80-$100 per senior citizen per month. Information must be uploaded to our system same day as your visit. Please let us know if you are interested.
