You can fill out an application at our office Monday - Fri 9 am to 3 pm . The application can not be removed from our offices . There will be two pages that require a notary seal ( Page 11 & 12 ) . Those two pages can be removed . Failure to return sheets will result in your application being filed as an incomplete application .When you are finished turn application in to our receptionist. In the event that you are unable to visit our office you can also visit one of our satellite locations. Our satellite location is open 24/7 . Our courtesy officers will provide one application per person & a photo ID is required.
