Summary:Works closely with Palliative Medicine & Supportive Care physicians and team in providing patient care in both hospital and office clinic setting. This position assists in providing patient consultation and evaluation; assistance with hospital rounds to include relaying of orders from supervising physicians; coordination of admissions and discharges. This position is responsible to provide documentation of services provided in compliance with established medical records, medical staff and billing procedures.This position is required to take call, including evenings and weekends, to support on call schedule (typically 1 out of every 4 weeks).The delivery of professional nursing care at Stormont Vail Health is guided by Jean Watson’s Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization. Essential Duties and Responsibilities include the following. Other duties may be assigned.

Utilize approved references and procedure manuals when providing care.

Perform focused bedside evaluation of patients.

Provide services related to health maintenance and promotion, palliative medicine and end of life care.

Assist with diagnosis, treatment and management of acute and chronic health conditions.

Order treatments and durable medical equipment as indicated.

Consult with Palliative Medicine & Supportive Care physician as needed; inform primary physician of services provided and collaboration with another physician if ordered.

Provide training and continuing education for staff and students.

Adhere to the practice of confidentiality regarding patients, families, staff and the organization.

Make daily inpatient rounds and record pertinent patient progress in charts.

Perform histories and physicals.

Write orders in hospital charts for review and counter signature by PMSC Physician.

Write standing orders as directed by PMSC Physician.

Records pertinent data on patient charts for signature by PMSC Physician.

Initiate consultation and monitor patients for special tests.

Dictate or write consultation, history & physical, progress notes and discharge summaries.

Prescribe medications and intravenous fluids.

Provide preliminary assessment and interpretation of various diagnostic studies.

Provide patient education and discharge instructions.

Arrange patient admissions and dismissals.

Communicates with families, employers, institutions and other healthcare providers regarding patient’s treatment and status.

Assumes on-call responsibilities as assigned.

Delegate tasks as indicated and appropriate to members of the interdisciplinary team.

Participates in department/interdisciplinary meetings and organization initiatives as directed

Executes other duties and tasks as assigned

Ensures that care delivered to patients is provided according to the Stormont Vail Health mission, strategic plans, and all internal/external regulations, policies and procedures. Practices within the Kansas Nurse Practice Act and within the guidelines of management protocols. Supports the established care models of Stormont Vail Health.

Has prescriptive authority per Kansas Nurse Practice Act and SVH protocols.

Qualifications:

Meets educational preparation or other qualifications within specified time frames as determined/outlined by specific department policy.Certificates and Licenses:Current RN and APRN or Physician Asst licensure in State of Kansas. Be certified as a nurse practitioner or physician assistant by a recognized national certifying body that has established standards for nurse practitioners/physician assts.Additional certification may be required in certain patient care settings or populations. The department director will provide specific information and the time frame in which such certification may be attained at the time of hire/transfer into department. Must be ACLS qualified or obtain prior to assuming ca11 responsibilities or within six months of employment, whichever comes first.
