The Manager Care Coordination oversees the implementation of team-based care coordination to provide health services to individual members, through effective partnerships with their caregivers/families, community resources, and their physician(s). The Manager Care Coordination facilitates a 'shared goal model' within and across the continuum ensuring that the member's needs and preferences are known and communicated at the right time, to the right people. Using this information to guide the delivery of safe, appropriate, and effective care, different levels of Care Coordination activities are provided to help members regain optimum health and self-management. The manager oversees service levels delivered to members by ensuring that department processes provide a comprehensive assessment of the member's conditions and care needs, determining available benefits and resources, while developing and implementing a plan to meet their clinical and behavioral health goals. By definition, care coordination necessitates communicating the patient's needs and preferences at 'the right time to the right people.' The information must be shared and used in a secure manner and in a way that provides 'effective care to the patient.'Essential Responsibilities:

Develops and implements integrated care coordination and case management models to promote an increase in communication and collaboration between care settings for patients.

Manages the regional care coordination and case management processes and programs to minimize variability and capitalize on leading practices among care coordinators and their partnering physicians.

Builds a clinical infrastructure by utilizing evidence-based practice guidelines to implement care plans for individual members and his/her care giver(s) to ensure that a high-level, integrated and personalized care is provided.

Uses technology and people to reach out to and engage providers throughout the continuum and hospitals, pharmacies, home health, and caregivers to ensure that patient information is readily available and updated as care levels and care settings change.

Responsible for working with the department to identify risk categories and develop, implement and evaluate programs related to primary, secondary and tertiary prevention to individuals and groups in schools, workplaces and other venues that focus on members with emerging risk, and at risk of disease and the management of chronic diseases.

Serves as an advocate for the patient and family as well as the health care system in all matters to ensure optimal and appropriate of use of health care resources.

Ensures that the Disease and Condition Management programs and operations are proactive, multidisciplinary, and part of a systematic approach to health care delivery that includes all members with a chronic disease; supports the provider-patient relationship and plan of care; optimizes patient care through prevention and proactive interventions based on evidence-based guidelines; incorporates patient self-management; continuously evaluates health status; measures outcomes; and strives to improve the overall health and quality of life for the member while lowering the costs of care.

Influences staff, physicians, and leadership with knowledge regarding the value and appropriate use of programs. Integrates and leads a diverse team of nurses, social workers and other clinicians who work in a variety of settings with a diverse patient population and often-conflicting demands.

Collaborates with leaders of other population management programs at Kaiser Permanente to ensure a well-integrated and seamless process and system for staff and providers wishing to access this model of care for their patients. Supervises assigned staff to include interviewing, selecting, training, motivating, evaluating, counseling, disciplining and terminating in compliance with EEO/AA goals and personnel policies of the organization.

Provides multi-disciplinary case consultation as needed. Collaborates with researchers and other program support to demonstrate value while streamlining the care experience.

Assesses areas of need for improved staff education related to care coordination. Plans and coordinates continuing education for program personnel.

Assesses and develops recommendations for new or revised departmental policies, procedures, and guidelines. Networks within the organization and the community to identify resources and information critical to the care coordinators.

Basic Qualifications:Experience

Minimum five (5) years management experience in multi-faceted healthcare setting (e.g. health plan, ambulatory, hospital) demonstrating experience and skills associated with change management and performance improvement required.

Minimum three (3) years Care Coordination, case management or disease management.

Education

Bachelor of Science Nursing degree required.

License, Certification, Registration

Unencumbered Active Colorado or in a reciprocal Compact State Licensure as a Registered Nurse.

Additional Requirements:

Demonstrated extensive knowledge of program design, business and clinical operations, management practices, principles, and trends, and organizational development trends and practices.

Strong skills in personal computer applications, database, and spreadsheet applications.

Exceptional interpersonal, verbal and written communication skills required to work with internal and external customers.

Preferred Qualifications:

Clinical operations experience in health plan with a focus on population management and integrated care coordination preferred.

Care Coordination and Transition Management Certification (CCTM) at time of hire or completed within 24 months after hire preferred.

Master's degree in nursing, health care administration, or related field preferred.
