Job Description

This position is under the supervision and supports the Case Managers by providing technical assistance to ensure implementation of discharge arrangements for all patients. Interfaces with external agencies such as nursing homes, medical equipment providers and insurance companies to facilitate a patient’s move to the next level of care, regarding discharge resources and financial resources.

PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:

  • Develops and maintains an ongoing directory of all resources essential for discharge including, but not limited to: Nursing homes (all levels), rehabilitation hospitals, chronic care hospitals, shelters, respites, other extended care facilities, day programs, home health services (tertiary, secondary, non-profit, for profit, etc.), durable medical equipment services, community agencies/services, emergency response systems, transportation services, entitlement programs, etc.
  • Collaborates with Financial Coordinators and Case Managers to identify appropriate financial resources/entitlements and assists patients/families with application process.
  • Under the supervision of Case Managers, will communicate with payer regarding Discharge service benefits Maintains daily contact with managed care providers and other third party payors, providing clinical information and requesting authorization of discharge services.
  • Under the supervision of Case Managers, will initiates referrals for rehab and long-term hospitals for all payers indicated.
  • Develops and maintains a centralized nursing home calling system to determine bed availability.
  • Identifies and arranges appropriate resources necessary to expedite discharge from Tufts MC(e.g., transportation, homemaker, DME, Discharge envelopes)
  • Work collaboratively with Social work Services to expedite discharges

Qualifications

JOB KNOWLEDGE AND SKILLS:

  • H/S diploma or equivalent; At least two years experience in clinical office management or discharge planning environment. Associates Degree preferred
  • Excellent communication and interpersonal skills; must be able to identify and negotiate boundaries of accountability and authority with multiple disciplines. Bilingual Skills preferred
  • Must demonstrate independent initiative and the ability to follow-up on details.
  • Must have excellent organizational skills and be detail oriented.
  • Must demonstrate high energy level; ability to prioritize demands and the ability to perform multiple tasks simultaneously.
  • Ability to work in an automated office environment.

ORGANIZATIONAL RELATIONSHIPS:

  • Reports to the Manager, Case Management
  • Collaborates on a daily basis with Case Managers, Professionals Nurses and social workers.
  • Collaborates with outside sources essential to discharge.

WORKING CONDITIONS:

  • Normal office working conditions
