Must be a certified nursing assistant, have successfully completed the
certified home health aide or personal care homemaker program OR can
show proof of having one years experience in the field.
Be at least 18 years of age
Have proper documentation to work in the U.S.
Have access to a vehicle and a valid drivers license
Pass a CORI, SORI and Drug test
Has a positive attitude and works well as a team
Responsibilities:
Provide personal care services for clients in a professional, caring
and efficient manner and in accordance with state guidelines
and per the clients care plan created by the Case Manager
Document all care appropriately (i.e., communication logs, incident reports, etc.)
Review communication log and discuss any pertinent
information regarding client at shift change
Report to Case Manager for client concerns
