We are seeking a private-pay social worker (or similar discipline) to act as a case manager for our male family member for an indeterminate period of time, most likely 3-6 months. We expect this to be anywhere between 0-10 hours on a given week.
We are primarily looking for someone to assist our family member in applying for Section 8, Disability, food stamps and Mass Health. The position may expand to other services, like securing housing, job training, transportation, etc if the partnership is going well.
Compensation:
To be determined based on services provided. Will be in line with industry standards
Requirements:
MA State Licensure (could be supervised if not independently licensed)
Experience with state systems such as welfare, housing, MassHealth, etc.
Excellent organizations skills
Case Management Experience
Duties:
Visits to the home and agencies within the nearby community
Assistance with applications/forms for Section 8 Housing and other housing programs such as Group Homes
Assistance with applications/forms for government programs such as DMH, Foodstamps, job trainings
Transportation of family member to and from interviews, agencies etc
