GENERAL SUMMARY:Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team.EDUCATION/EXPERIENCE REQUIRED:

Bachelor's degree in Nursing preferred.

Organizational and Interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families.

Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.

Analytical ability and motivation necessary to effectively plan, deliver and implement plans of care to in acute and outpatient situations.

For those working as a Coordinator of Heart Transplant/with VADS: Experience in ICU or OR preferred.

CERTIFICATIONS/LICENSURES REQUIRED:Current RN license in the State of Michigan.Overview

  • The Specialty/Invasive APP will work in a collaborative, multidisciplinary environment. The Specialty/Invasive APP will be responsible for the assessment, diagnosis, treatment, education, consultation, and care coordination as a first line provider to patients and their families. Provides inpatient and/or outpatient medical care. Works in collaboration with attending physicians, advanced practice providers, consultants, house officers, nurses, and other members of the medical care team to ensure that patient outcomes are achieved safely and efficiently. Specific procedures as per credentialing as indicated by training and experience. Works in one of the following departments: Cardiovascular Medicine, Emergency Medicine, ICU, Interventional Radiology, Nephrology, Neurology, Neurosurgery, Oncology, Orthopedics, Otolaryngology, Pulmonary and Critical Care Medicine, Surgery, Urgent Care, Urology. Requires Michigan state licensure and continued certification by the National Commission on Certification of Physician Assistant (NCCPA).-OR- Requires master\'s degree in nursing; licensure as a Certified Nurse Practitioner; licensure as a Registered Nurse Current certification as a Certified Nurse Practitioner. Current State license as a Registered Nurse. Current BLS. Current ACLS/ATLS/PALS provider status as required by department.
