The Registered Nurse’s position requires the occupant to hold a current Minnesotan license as a registered nurse. Experience in client case management in home health care or in a related field (community-based, assisted living, or nursing home, etc) is desirable. This visit-wage position is ideal for clinical field professionals who desire non-office-based work schedule. RNs conduct home visits with clients and complete relevant patient charting in a web-based electronic health records (EHR) database.
Client case management involves:
o Care planning, coordination, and implementation;
o Oasis assessment and intervention/outcomes monitoring;
o Physician notification and follow-through on MD orders;
o Disease prevention to include client/family caregiver education;
o Care documentation into patient charts (Electronic data entry into Axxess database); and
o Chart audits for timely/thorough/accurate documentation of authorized services
