REM Minnesota, a member of the Sevita family, provides services for Individuals with intellectual and developmental disabilities, Traumatic Brain Injury, Autism, and other complex needs.Position Summary REM Minnesota, a partner of Sevita Health is seeking a passionate leader to be a part of the REM Minnesota Statewide Development Team. REM has partnered with the Minnesota Department of Human Services to participate in the Transitions to Community Initiative to develop unique discharge plans to individuals currently at AMRTC or MSH-St. Peter who are unable to be discharged due to individually-specific barriers. The identified individuals have complex needs that complicate discharge planning which may include, but are not limited to, co-occurring substance, physical, or cognitive disorders, developmental disabilities, aggressive behaviors, high acuity of symptomatology, refractory mental health symptoms, traumatic brain injuries, Mentally Ill and Dangerous (MI & D) status, or other legal issues that have led to the need for a high level of intensive services that cannot be met by current community services. Key Responsibilities The Reintegration Specialist with work with the Director of Mental Health Services to support transitioning individuals from hospital settings into the community. Responsibilities of this position include but are not limited to creating unique and creative discharge plans for individuals who require a high level of care. This position will review referrals, meet individuals and create person centered plans, determine level of service needed, secure housing, work with providers, work directly with individuals, and obtain other resources identified for individuals needing supports. This position will require extensive travel and work with other service providers throughout the state. Requirements 18 years of age or older.

A bachelor’s degree is required in one of the behavioral health or allied fields (such as, but not limited to; criminal justice, mental health, special education, or social work) from an accredited college or university.

A master’s degree is preferred in one of the behavioral health or allied fields (such as, but not limited to; criminal justice, mental health, special education, or social work) from an accredited college or university.

A minimum of three years of related work experience within the human services industry with an emphasis in working with SPMI.

Skilled in written and oral communication, community relations and service development.

Demonstrated knowledge and understanding of behavior management principles, conducting assessments, writing plans and providing recommendations.

Eligible as determined by the Bureau of Criminal Apprehension, the Department of Human Services, and the Company’s Corporate Compliance Plan.

Valid driver’s license and driving record that meets driving record requirements as defined by the Company.
