In our residential settings, we work together with our clients to offer the support they need in order to succeed. Our facility is seeking an experienced Licensed Practical Nurse (LPN) to lead our Home Care Mission and bring our clients even greater success.
Duties include but are not limited to:
Handling MARS, medication changes and medication list management
Maintain ongoing communication with clients, physicians, families, staff and other responsible parties
Making and taking medical related phone calls
Perform and oversee the maintenance of all required documentation, charts and clinical notes
Ensure compliance with all State statutes and rules and Company policies and procedures
Providing oversight and review of client medical issues for 6 clients and perform a wide range of nursing services to clients
Qualifications:
Current Licensed Practical Nursing license in the State of Minnesota required
One or more years of experience working with mental and physical disabled population
Supervisory or managerial experience preferred
Valid Minnesota drivers' license with good driving record, proof of valid insurance and have reliable transportation
High level of compassion, customer service and communication skills to interact with clients, family members and coworkers
Able to read, write and speak English fluently
Computer skills and experience required, including Microsoft applications such as Word, Excel, Outlook, and applicable clinical software
Needs to have flexibility and be reliable and dependable
Strongly prefer candidates to have experience with mental health
Position is part-time, 15-20 hours a week with flexible hours.
