Job Description:Provides a lead role in a specified geography helping generate, disseminate, and coordinate key messages and best practices to providers contracted with Castell. This role supports the efforts of a local geographic committee and is expected to drive execution on population health strategies by working effectively with clinic managers, staff, and physicians. Oversees and coordinates the work of various groups throughout the system from affiliated clinics and providers, SelectHealth, third party payers, Castell Operations, and others. Ensures access to data, resolves provider concerns, holds clinic groups accountable, and builds provider engagement and loyalty.ScopeOversees the provider relations for a geographic area and responsible for provider relations with multiple provider stakeholders. Develops Population Health performance in value-based care.Job EssentialsActs as the point person within an assigned Castell Geographic area to collaborate with affiliated clinics to support the needs of their assigned population.Develops and maintains effective business relationships with providers. Maintains a system to visit each practitioner and participating providers on a regular basis. Functions as an intermediary between providers of the network and the organization.Participates on regional and or corporate committees such as Geographic Committees and their corresponding workgroups.Develops and manages a system of regular visits to target providers to educate them about Population Health products and clinical programs, and review reports relevant to their practices.Understands multiple data points relevant to population health and is comfortable discussing performance with clinic and providers.Navigates data tools to understand actionable clinic and provider opportunities.Serves as a liaison between Castell and geographic clinic relationships concerning Population Health topics.Furthers education about risk adjustment, cost, and quality topics with practitioners and their office staff, reviews management information reports, and functions as a liaison.Provides continuous orientation and training for all participating provider business office staff.Manages the development and maintenance of an effective and continuous communication program for a large number of providers who have limited knowledge about population health.Manages the development, maintenance, and distribution of all marketing materials that assist providers to interface more efficiently with the administrative processes.Manages the formulation of policy regarding systems and activities; the level and nature of integration with the practitioner community.Provides a continuous evaluation of the communication system and recommend design and delivery method changes.Supports education about network participation criteria and helps educate providers and clinics on how to be successful in various payment models.Responsible to help coordinate any issues or concerns that may arise between the clinic and Castell. Works to ensure clinics are delivering the highest possible care and has appropriate interoperability with Castell.Minimum QualificationsBachelor's degree in a business-related field. Degree must be obtained through an accredited institution. Education is verified.or -

Five years of experience in a management or a professional level position.Five years of professional experience in the health care industry.Preferred QualificationsExperience in value-based care delivery and/or population health services.and -

Ability to think and communicate strategically on various health care related topics.and -

Effective presentation skills.and -

Strong interpersonal and problem-solving skills.and -

Computer business applications proficiency.and -

Health insurance experience or experience working in a clinical setting.Physical Requirements:Interact with others requiring the employee to communicate information.and -
