The Youth and Adult Services Coordinator/Community Support Worker (SC/CSW) will provide case management services to youth and adults with severe and disabling mental illnesses under the supervision of the site manager, the Program Supervisor and Executive Director. SC/CSW will assess and mutually determine a treatment plan with participants, schedule and invite team members to treatment team meetings, monitor treatment plan implementation, adjust goals as indicated, and mutually establish criterion for discharge with participants. SC/CSW directly supervise participants during recreational outings and social activities and supports the development of coping skills. SC/CSW will also provide illness, management and recovery skills groups and other recovery focused groups, as requested. SC/CSW services will be provided to assist youth and adult participants to gain access to needed medical, emotional, social, educational, vocational, recreational, and other services as requested or needed. The SC/CSW also provides emotional support to participants to assist their maintenance in the least restrictive setting and to enhance self-sufficiency skills. The SC/CSW provides transportation for the participant related to outings and resource connection. The SC/CSW communicates emotional and behavioral observations to the treatment team and completes record keeping functions. Caseload and/or billing expectations and requirements must be met on a monthly, quarterly, and yearly basis. The employee will be required to follow any other job related instructions and to perform any other job related duties requested by his/her supervisor.Education/Experience: Bachelor?s Degree in Human Services, Social Work, or a related field Minimum of two years of direct service with children, families or adults Experience with resource/referral and working with teams Minimum qualifications for a case manager are a bachelor's degree in a human services field with at least one year of full-time experience serving people with mental illnesses. Individuals with other educational backgrounds who, as providers, participants, or advocates of mental health services have developed the necessary skills, may also be employed as case managers. Equivalency provisions for SCs who do not have a bachelor?s degree in a human services field include: experience and skills must be reflective and demonstrative of having been a provider, participant or advocate of mental health services. SCs with mental health services must provide sufficient background to support 5 years of above experience and skills. Job Overview: The Fire Adapted Communities Coordinator (FACC) primarily focuses on leading the development and implementation of a unit level program for community preparedness, fire prevention, fire information, and public outreach supporting both the current operational needs and the strategic initiatives of their Land Office, the Fire Protection Program, and the Forestry Division. The Libby Unit FACC operates under the supervision of the Libby Unit Fire Management Officer (FMO). Areas of emphasis include fostering fire adapted communities and promoting community preparedness, reducing human caused fires, supporting Libby Unit and Northwest Land Office (NWLO) efforts to create and maintain resilient landscapes using prescribed fire and hazardous fuels reduction, and educating the public. The incumbent may be requested to provide fire information and function in an ICS position or support role during emergency situations and during special projects as assigned. Work assignments are covered by established policies and general program objectives; and the incumbent is responsible for working with the FMO to jointly determine the methods and techniques necessary to carry out assigned responsibilities. Essential Functions (Major Duties or Responsibilities): These job functions are the essential duties of the position and are not all-inclusive of all the duties that may be assigned to the incumbent. Lead and Manage the Unit Community Preparedness Program: Serve as the principal subject matter expert and point of contact at the Libby Unit regarding matters pertaining to community preparedness, fire prevention, fire information, and public outreach.
