Responsible for providing case management for children and adults with intellectual or developmental disabilities or delays. Ensures quality services are provided in accordance to all agency or government regulations.What would you do in this position?
Complete required paperwork and maintain accurate records in department or government database
Communicate professionally and positively with others
Schedule, attend, and facilitate in-person and virtual meetings as required by the department
Maintain ongoing contact with family and providers
Coordinate, monitor, and change services
Assist with the transfer or coordination of services and referrals and follow-up as needed
Coordinate referrals and follow-up as needed
Maintain productivity standards as required by agency or government regulations
Assess eligibility and benefits and take action as necessary
Translate documents and communications in both English and Spanish as neededQualifications
What education or experience would you need?
Valid Drivers License required
High school diploma or equivalent required
Bachelor's Degree in a Human Services field preferred
5 years or more experience working with people with disabilities required
Combination of education and experience may be used to meet the education and work experience requirementsWhat knowledge, skills, or abilities would you need?
Ability to communicate clearly in oral and written forms
Ability to use office equipment such as fax machine, scanner, copier, phone
Ability to work a flexible and remote work schedule independently
Ability to work as a team
Analytical thinking skills to solve routine problems using common sense
Basic knowledge in Microsoft Word, Excel, and Outlook
Basic knowledge of state regulations
Basic knowledge of technology such as internet browsers, PDF software, database systems
Basic organization and time management skills
