Job Description

  • Rapidly (within 1 week) conduct intake with newly referred youth and families to review program expectations, create youth schedule, co-develop success plan and conduct outreach to necessary referrals
  • Cultivate professional, responsive, and pro-active relationships with youth referred to PEAK and provide ongoing support to participants referred to, enrolled in and who are alumni of the PEAK program
  • Develop strategies with youth to make progress on their success plans
  • Effectively refer to and collaborate with Case Coordinators, Adolescent Portable Therapy (A.P.T.) Therapists, High School Equivalency (H.S.E.) Teachers and stakeholders to identify opportunities for increased efficiencies, new opportunities for social, financial, and educational advancement and provide escort as necessary to community/court services
  • Act as a liaison, maintain communication and conduct accurate record keeping regarding youth engagement and reporting from providers and probation officers
  • Ensure the timely flow of community outreach and case management engagement to meet program goals and objectives and report issues to Program Director proactively
  • Ensure the timely entry of all data and documentation including weekly and monthly progress reports for every youth on caseload to be shared with probation officers
  • Review, categorize and upload documents and maintain updated information in Salesforce systems and designated Share Doc databases
  • Provide all services in a manner that is gender-responsive, trauma-informed, and that adheres to the core principles of risk-need-responsivity
  • Participate in client conference calls, court dates and/or meetings as necessary
  • Communicate with probation officers to support compliance for all probation requirements
  • Collaborate with families and other supportive people to ensure youth are fully supported
  • Conduct outreach and recruitment at probation
  • Conduct in-community visits, facilitate celebrations and lead youth outings
  • Develop creative ideas and group lesson plans and workshops for engaging youth
  • Participate in team meetings and case conferences as well as internal and external training’s that focus on skill-building and professional development
  • Compile and report all timekeeping activities accurately in ADP time keeping system
  • All other duties as assigned by Program Director

Qualifications

Bachelor’s Degree or Equivalent Experience

Bilingual (English/Spanish) Preferred

Experience working with youth, preferably court-involved youth

Proficient in Microsoft Office, Excel and Outlook

Experience facilitation of groups

Excellent verbal and written communication skills

Analytical and creative reasoning skills

Detail oriented

Demonstrated initiative and independence

Self-motivated and directed

Strong customer service skills

Experience managing and prioritizing multiple cases, tasks, projects, and deadlines

Willingness to remain calm under stressful situations

Ability to comply with privacy and security policies and procedures

Ability to apply youth development principles and commitment to work from a strengths-based perspective

Passion to work with and support the positive development of youth and young adults

Commitment to working with court-involved young people and a thorough understanding of the complex systems, issues and factors that affect them

Salary: Competitive compensation, commensurate with experience
