Summary Statement:

Working within a community-based behavioral health setting, the ACT Clinician works to provide effective and quality clinical services, coordinates and monitors other services provided as needed, promotes independent functioning and develops self-sufficiency skills for CLM clients and their families. ACT is a treatment model that covers for 24/7, 365 days of the year. This is a full time, thirty-five hours a week position which could include weekends, evenings and holidays.

Major Functional Areas and Description of Duties:

Provides individual treatment plan assessment and development, crisis management planning, crisis interventions;

Provides individual and group therapies.

Provides outreach, advocacy, resource identification, referral, mobilization, linkage, monitoring and coordination of services activities with clients and in collaboration with other team members;

Coordinates medical services and emergency services as needed;

Participates as a member of the clinical service multi-disciplinary treatment team and serves on appropriate committees, both internal and external, as determined by the supervisor;

On call after hours coverage on a rotating basis.

Holiday coverage will be shared by team members.

Executes all required services and documentation in accordance with agency policies and federal and state standards;

Represents CLM in a professional and appropriate manner in the community;

Performs any additional duties assigned by the supervisor.

Provides on call coverage periodically.

Qualifications and Skills:

Prior experience and thorough knowledge of recovery oriented service delivery and clinical functions;

Demonstrated ability to be self-directed and motivated with minimal supervision;

Demonstrated ability to work as an effective member of a team;

Proficient computer skills, specifically with all Microsoft office applications.

Ability to communicate effectively and compassionately with clients and their families;

Ability to communicate effectively and collegially with other professionals across disciplines, inside and outside of the agency;

Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.

Education and Experience:

A Masters degree in social work or a similar discipline is required, along with a minimum of two years of direct service mental health experience.

Physical Demands and Working Conditions:

A majority of the work will be visiting clients in the community and also in their homes. Proof of car insurance, a reliable vehicle and a clean driving record all are required. An ability to freely enter/exit clients homes is necessary.
