The Outreach Worker will work directly with individuals who are deaf and hard of hearing in the community, to network and increase communication regarding available services and programs.
Provide outreach in the community to bring new participants into programs
Maintain and organize attendance and applications
Review applications for missing and/or incomplete paperwork
Excellent Communication skills/ Must be able to communicate via ASL
Must be organized and able to multi-task
Must be able to work with diverse cultural, educational, social and economic backgrounds
Must have reliable transportation and a valid Driver's License
Responsibilities
Work alongside Case Management team to:
Gathers pertinent information about the referred consumers, to include demographic, source information and completes written referral form.
Assist in the applicant process for financial entitlement programs, Medicaid, Medicare and other health insurance programs.
Ability to explaining program requirements and procedures.
Providing community referrals, linking clients to resources.
Keeps an intake log of all referrals with outcomes.
Assist with scheduling as needed.
Entering data into internal and external databases
Maintain clear records
Other duties as assigned to meet the needs of the case management department.
