A + Senior Care is also seeking a Per Diem Registered Nurse to Promote and establish quality home care services by completing the nursing process defined by the State of NJ as it relates to Health Service Firm nursing Coordinator position. Collaborate with other health care providers involved in or who are part of the patient's care team, Provide psychological support to patients, and families, supervise, direct, and train Certified Home Health Aides.
Registered Nurse Job Duties:
Identifies patient care requirements by establishing personal rapport with actual patients and completing a comprehensive care plan.
Establishes a compassionate relationship by providing emotional and , psychological, support to patients, friends, and families as needed
Promotes patient's independence by establishing patient care goals; teaching the Caregiver to understand condition, needs, and skills; answering questions.
Assures quality of care by adhering to State standards, making or recommending necessary adjustments in care plan as you deem necessary.
Know when to call for assistance from support personnel.
Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
Documents patient care services using Generations software, plan of care tool, reassessment tool and other necessary resources.
Maintains continuity among team by documenting and communicating actions, irregularities, and continuing needs of the patient.
Maintains patient confidence and protects operations by keeping information confidential.
Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
Maintains a cooperative relationship among outside health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
Contributes to team effort.
Must complete an initial care assessment, a 30 day phone assessment, and a 60 days reassessment of the patient.
Provide training and in-service to employees both in the home and at scheduled site/s
Skills/Qualifications: Clinical Skills, Customer Service Manner, Nursing Skills, Training , Home Care Background, Supervisory Experience, Administering Medication, Medical Teamwork, Multi-tasking, Listening, Verbal Communication.
Requirements:
Must be a Registered Nurse with BSN
Must have at least one year experience in home care
