New Vistas is seeking two full-time (40 hours/week) Bilingual Family Service Coordinators (FSC) in Santa Fe County. Open until filled.

Family Service Coordination services are carried out to assist and enable the families of children from birth to three, to access, and if determined eligible, receive early intervention services. The Family Service Coordinator helps to develop the Individualized Family Service Plan (IFSP); assists the family in receiving all services identified; coordinates those services; ensures that they are delivered in a timely manner, and seeks additional services and or supports that may help the child or family. These services will be conducted within a variety of environments in home and community settings.

Essential Duties and Responsibilities

The FSC will operate within State and Federal Regulations as well as adhere to all New Vistas policies and procedures. Essential duties include but are not limited to the following:

Explain to families the early intervention process, parent rights, and safeguards

Gather information from the family regarding their concerns, priorities, and resources

Coordinate the evaluation and assessment activities

Facilitate the determination of the child’s eligibility

Connect the family with resources and supports

Help families plan and prepare for their IFSP

Organize and facilitate Transdisciplinary Team Approach (TTA) and IFSP meetings

Arrange and coordinate all services listed on the IFSP

Monitor the delivery of services

Facilitate periodic reviews of the IFSP and ensure that progress is being made toward family goals

Provide families with the information and support to enhance their child’s development in areas including cognitive, communication, physical/motor, social or emotional, and adaptive development

Manage and coordinate the transition process beginning with the transition plan and finalized at the transition conference

Ensure that the initial visit is face-to-face and at least one face-to-face visit occurs each month

Home visits, community visits, and travel are required

The minimum requirements for the advertised Family Service Coordinator position is a bachelor's degree in social work, counseling, psychology, nursing, special education, early childhood education, or a closely related field, plus one year of related experience. Additionally, selected candidates must be able to pass a required criminal background check, driving record check, and credential verification and must possess reliable transportation. Bilingual proficiency in English and Spanish are highly preferred, with birth to three and home visiting experience being a plus.

New Vistas offers a competitive hourly rate of $17.51/hour for a bilingual FSC plus benefits. New Vistas will conduct an assessment for bilingual proficiency and verify credentials with all final candidates. The hourly rate will be negotiable depending on experience, bilingual proficiency, and level of education. The schedule will be moderately negotiable for the candidate offered the position and the majority of work will occur Monday through Friday.
