OverviewAt CRJ we change lives and strengthen communities by advancing policy and delivering individualized services that promote safety, justice, and inclusion.

ResponsibilitiesProvide comprehensive reentry services to a caseload of individuals with higher mental and/or medial health needs and ensure that program participants are accountable for their whereabouts by conducting site visits to different community locations.

Functional Responsibilities:

The employee will maintain a caseload of 10 – 15 program participants. These program participants will reflect those deemed to have higher needs as it pertains to mental health and medical issues.

The employee will conduct one-on-one meetings with program participants, at a frequency dictated by relevant contracts.

The employee will conduct group counseling sessions with program participants. The employee will use the Thinking for a Change curriculum.

The employee will complete all documentation associated with the role of a Case Manager. Examples of such documentation will include the completion of Individualized Reentry Plans, Case Notes, and other forms of written correspondence with stakeholders.

Coordinate services for program participants through an approved set of vendors.

The employee will travel to community locations for the purposes of enhancing the program's security and accountability. These sites will include employment sites, home detention sites, and other community locations as designated by the Program Director or designee.

The employee will travel to community locations for the purposes of conducting site inspections.

Maintains vehicle records and general maintenance of personal vehicle.

The employee will participate in an on-call rotation, at a frequency to be determined by the Program Director.

The employee will attend all internal meetings and trainings. Additionally, the employee will attend all external meetings and trainings, as required.

RequirementsQualifications

Bachelor's Degree in Criminal Justice, Social Work, or other applicable Human Services field.

1 –year of experience in working with a high needs population, preferably in a residential setting.

1- year Education and/or professional experience in working with individuals afflicted with mental health disorders.

Valid New York State Driver's License

Additional Information

As this position requires travel, a company vehicle or mileage/fuel reimbursement will be provided.
