Location: Remote – Must Reside in California; travel 3-4 days per week to hospitals and provider offices; overnight stay within the State of CA on a quarterly basis, travel can be consolidated by consultant.The RN Practice Consultant is responsible for creating a partnership relationship with providers and other initiatives to achieve this fully integrated and accountable system of care.Primary duties may include, but are not limited to:

Assesses current practice patterns, assets and challenges utilizing available EPHC and non EPHC quality data.

In partnership with the PCP and the local provider solutions team, evaluates results of the initial assessment.

Establishes a project plan with the practice that establishes goals and objectives for HEDIS and CAHPS improvement.

Provides ongoing practice support and sustains the partnership.

Establishes regular meetings to track progress.

Support provider outreach to review health plan quality measures and to implement quality outreaches for provider groups.

Qualifications

Must have a current, unrestricted California RN license

Requires an AS/BS in Nursing

5 years of managed care experience;

2 years of clinical experience;

2 years of professional presentations to small and large audiences; or any combination of education and experience, which would provide an equivalent background.

Medical office operations experience

LEAN/Process Improvement experience, preferred

Experience with HEDIS and CAHPS focused quality improvement, hospital and provider experience strongly preferred.

This position requires local travel 3-4 days a week.

BSN or MSN preferred.
