Job Summary
Allied Community Support Services, a nonprofit organization, is looking for a caring and dependable Case Manager to join our team. In this role, you will work directly with clients to make sure they receive the support and services they need. The job includes meeting with clients, checking on their care in nursing homes and hospitals, writing progress notes, and traveling to different locations. We welcome candidates with experience in social work, case management, or legal settings.
Key Responsibilities
Meet with clients to assess their needs and help create simple care plans
Write clear progress notes after client visits and phone calls
Visit clients in nursing facilities or hospitals to ensure they are receiving proper care
Communicate with healthcare providers, family members, and others involved in the client’s care
Talk with clients about their needs and advocate on their behalf
Travel locally to meet clients and attend appointments (company vehicle provided)
Keep organized and up-to-date case files and documentation
Skills and Qualifications
Bachelor’s degree in Social Work, Psychology, or a related field preferred
Experience in case management, social services, or a legal environment is helpful
Strong organizational and time management skills
Comfortable balancing multiple cases and deadlines
Basic computer skills, including Microsoft Office and case management software
Strong communication and people skills
Able to handle sensitive information responsibly
Valid driver’s license and willingness to travel
