Concern for Independent Living, Inc. is a non-profit agency committed to helping individuals and families to live in the community with dignity and enhanced opportunities through the provision of housing and support services. We are one of the largest housing agencies of this kind in New York State, currently serving approximately 1,300 individuals and families in over 240 locations. Concern offers a variety of housing options with individualized support services designed to support personal growth and independence.
Concern for Independent Living is seeking a Wellness Specialist who will be primarily responsible for providing linkages with community resources, daily living skills training, counseling and support, documentation, and compliance with program requirements. The Wellness Specialist is an essential resource and advocate for their clients.
Duties and Responsibilities:
â¾Facilitate residents' linkage with services available in the community (e.g., case management, clinical services, day programs, vocational services and opportunities, community integration, entitlements, etc.).
â¾Development of comprehensive needs assessments and support plan. Meetings with clients to develop skills and review progress. Responsible for all documentation required for client record (e.g., progress notes, discharge plans, etc.).
â¾Provide individual counseling and support services.
â¾Provide medication management services
â¾Engage in recovery planning through the use of Wellness, Recovery and Action Planning
â¾Provide daily living skills training (e.g., medication monitoring and training, cleaning, money management, etc.).
â¾Attendance at supervision and staff meetings.
â¾Remain current with all Agency required compliance documents and trainings.
â¾Other duties as assigned by the Team Leader.
Qualifications: Bachelor's degree or higher in Social Work, Psychology or other related field as well as a minimum of 2 years experience in the mental health field.
Schedule: Sunday - Thursday; 11am - 7pm
