Job Description:
Work with patients of all ages that require care in their homes
Educate patients and their families on proper home health care strategies
Supervise home health aides
Must provide each patient with the time and attention to meet their needs
Qualification:
Licensed Registered Nurse (RN) in the state of NY with no disciplinary action
3 year of work experience in field based eldercare/homecare
Experience working with the needs of vulnerable populations
Good communication skills and knowledge of community resources
Proficient with Microsoft applications: Outlook, Word, Excel, and PowerPoint
