Job Summary:
Under the direction of the registered nurse, provides nursing care in
accordance with departmental and hospital policies and procedures,
supporting the care plan established for each resident.Job Qualifications:
Education - Completion of an LPN program
Experience - Previous long term care experience preferred
Knowledge/Skills/Abilities - Ability to communicate effectively and
document information accurately. Ability to deal efficiently and
effectively with a wide variety of individuals. Knowledge of medical
terminology. Ability to move or lift residents and equipment
Physical Requirements - Stand/walk 6-8 hrs, sit 1-3 hrs, bend, squat,
reach, lift, carry, push, pull 1/3 or more of the time. Weight
lifted/force exerted up to 35 pounds.
Licenses/Certifications - Current NYS license to practice as an LPN. CPR
Certification required.Essential Functions:
-Transfers, moves and repositions clients every 2-3 hours and prn.
-Assists clients to move to activities area or dining area, etc.
-Keeps unit neat, relaxed and homelike.
-Assists with admissions, transfers and discharges
-Assists/provides residents with PROM and AROM, and ambulation goals as
appropriate.
-Provides treatments/medications as ordered
-Operates all equipment and performs all procedures/care in a safe
manner
-Demonstrates proper body mechanics in all functions.
-Maintains current knowledge/certification/licensure.
-Notifies appropriate persons/family members of significant events (in
conjunction with RN)
-Demonstrates ability to accept constructive criticism and uses it to
improve work performance.
-Completes work assignments on time/readily accepts assignments as
observed by RN
-Monitors performance of CNA's and relays concerns to the RN for follow
up.
-Attends all required safety training programs and can describe his or
her responsibilities related to general safety, department/service
safety, and specific job related hazards.
-Promotes effective working relations and works effectively as part of a
department/unit team to facilitate the department's/unit's ability to
meet its goals and objectives.
