HOURS: Full-Time 40 hours a week Sunday: OFF Monday OFF Tuesday: OFF Wednesday: 3:00 pm - 11:00 pmThursday 1:00 pm - 11:00 pm Friday: 1:00 pm - 11:00 pm Saturday: 8:00 am - 8:00 pm Number of hours subject to change based on department/participant needs. Main Function: With on-going supervision, provide person centered services and supports to assist the individuals residing at the IRA to achieve independence, and maintain their health and safety and stabile housing. Responsibilities include: assistance with personal needs and ADLs, monitoring health and safety, building advocacy, decision making and community connections, and completion of required documentation. Contact with public is moderate. Duties and Responsibilities:
Provision of services for participants
Implement participants’ goals as written in habilitation plan
Assist/support participants with ADL skills including, but not limited to, bathing, toileting, dressing, dining, mobility, as needed
Teach participants skills to promote independence including housekeeping, laundry, cooking, money management
Follow diet orders and prepare meals according to diet orders
Implement Individual Protective Oversight Plan (IPOP) and ensure safeguards are implemented correctly
Implement behavior plans as written
Offer choices and opportunities for participants to engage in and increase leisure skills and community activities and connections
Provide support to participants in building friendships and maintaining contact with family and friends
Support participants by providing opportunities to increase decision-making and advocacy skills
Transport and accompany participants to activities of their choice
Provide support to maintain the health of participants
Obtain and maintain AMAP certification
Administer medications and treatment plans correctly
Implement exercise, home therapy, nursing plans and positioning plans correctly
Follow diet orders and implement dining guidelines as written
Transport and attend medical appointments with participants as assigned. Communicate any recommendations/changes to supervisor and RN if applicable
Follow medical and nursing recommendations as directed and report and document changes as they arise
If applicable, obtain and maintain g-tube certification and follow g-tube procedures
If applicable, obtain and maintain Insulin administration certification and follow Insulin administration procedures
Obtain and record vital signs as directed
Monitor participants’ health conditions and report any changes/issues per Agency procedures
Offer participants choices and encourage them to make healthy choices
Monitor and maintain participants’ safety
Implement safeguards as written in IPOP
Assist with the completion of participant safety assessments as assigned
Conduct and participate in fire evacuation drills following the written evacuation procedure as assigned
Monitor and maintain the safety of participants in the IRA, community, and during transportation
Obtain and maintain CPR/FA certification and follow Agency procedures as need arises
Report participant incidents per Agency standard, including reporting to Justice Center if applicable
Utilize pro-active approach to avert and/or de-escalate crisis situations
Follow on-call procedures to report emergencies or medical needs/changes of participants
Completes required documentation
Complete all program documentation in a timely manner and per Agency standards. Documentation includes but not limited to: service notes, MARs and treatments, behavioral data, program outcomes, etc
Follows change of shift procedures and completed required documentation
Completes fire evacuation drill forms correctly and in a timely manner
Completes incident report forms thoroughly and in a timely manner per Agency standards
Maintains receipts from purchases and completes necessary documentation per Agency standards
Completes van maintenance and mileage sheets per Agency standards
Reports IRA maintenance needs and completes required paperwork as directed
Community and Teamwork
Work collaboratively with participants, families, program staff, co-workers and supervisor
Attend staff meetings and other meetings/committees as directed by supervisor
Attend all required trainings as directed by supervisor and maintain training certifications
Participate in Agency-wide events, advocacy, and/or community education.

Education/Experience: High school diploma/GED required. Experience working with people with disabilities preferred. Basic word processing and e-mail skills are required. Conditions of Employment: Must have valid New York State driver’s license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs. Must be able to lift 75 pounds. Successful completion of required orientation and annual training. Must obtain and maintain AMAP certification. Must be able to demonstrate teamwork and professionalism. Must be able and willing to demonstrate direct care duties, including personal assistance. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of People with Developmental Disabilities, Chapter 575 of the Laws of 2004. Must participate in successful database checks through the State Central Register of Child Abuse and Maltreatment, HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
