Ensure linkage of vulnerable refugees to appropriate medical or mental health clinicians, following the guidelines and timelines of IOM/PRM when identified before arrival. Ensure that vulnerable refugees consistently attend medical and mental health appointments. Arrange transportation as necessary. Provide supportive counseling and guidance as needed to help refugees acclimate and integrate into the community. Provide crisis intervention supports as needed. Link refugees to additional support services as needs are identified. Assist with maintaining housing, public benefits, employment if appropriate, educational programs, and other resources that will facilitate the refugees full integration into the Syracuse community. Facilitate a weekly support group for refugees, ensuring enrollment of a minimum of 15 participants. Maintain a caseload of at least 15 intensive case management clients. Maintain timely case notes, service plan reviews, assessments, and enrollments as outlined by USCCB/MRS. Maintain positive working relationships with medical and mental health providers in the community. Develop new partnerships as needed. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.Bachelor’s degree in Social Work, Psychology or human services related and two (2) years experience working with youth, individuals who speak English as a second language, refugees, and individuals of diverse backgrounds. Valid N.Y.S. Drivers License, N.Y.S.
