RN Nurse Navigator
Provides professional nursing care to patients at our outpatient infectious diseases practice as they transition from inpatient to ambulatory care. Specifically, the registered nurse navigator provides direct patient care and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with clinical research office, increase patient and provider satisfaction, maintain point of contact with providers and serve as the first point of contact for patients and families. NYS licensed/registered RN, current CPR certification through the American Heart Association or the American Red Cross required, previous RN experience in an inpatient setting or outpatient practice setting with Infectious Disease, Infusion or Procedural area preferred.
