Job Summary:

Under limited supervision, this person will provide support for individuals in a variety of ways such as assessment, job development, training, technical assistance, monitoring safety conditions, providing transportation and follow-along support on a daily basis in a community based environment.

Essential Duties and Responsibilities Summary:

Other duties may be assigned as necessary. Ensure individuals are referred to and have completed the Single Portal Process. Follow VR regulations and guidelines for supported employment. Exercise judgment in implementing goals, policies and procedures. Marketing to perspective employers in order to obtain job placements for individuals with disabilities. Must be available to work flexible hours including nights, weekends and overtime as required. Transport individuals to and from various locations in a safe vehicle while maintaining a confidential environment. Responsible for an average of 12 job placements per fiscal year. Must consistently meet quarterly revenue objectives. Submit accurate weekly administrative tasks by established deadlines. Assist in selecting potential individuals through screening and assessment procedures. Oversee and direct the activities of individuals while performing job duties. Facilitate onsite job instruction, ensuring that they are placed in the most appropriate and least restrictive job for their individual needs and interest.

Knowledge, Skills and Abilities:

High school diploma or general education degree (GED) is required for this position. Two years of experience in working with individuals with disabilities or related experience is required. Ability to communicate clearly expressing ideas and thoughts verbally and in writing; Exhibits good listening and comprehension. Establishes and maintains effective relationships; develops collaborative relationships. Responds to individuals concerns with accurate information in an effective and timely manner while working in a team environment. Follows policies and procedures and strive to continuously build knowledge and skills. Demonstrates accuracy and thoroughness. Ability to withstand heavy lifting and the ability to transport individuals to various locations is required. Ability to document and communicate pertinent information using computer and/or paper documentation tools. Knowledge and understanding of telephone etiquette. Working knowledge of general clerical practices and procedures. Ability to apply specific department rules and regulations relating to the care of the individuals. Working knowledge and ability to use technology to include computers, scanners, communication systems and other systems. Ability to deal tactfully and courteously with a variety of individuals and groups. Working knowledge of LIFESPAN CHOICES Training Curriculum. Ability to maintain a valid driver's license, reliable transportation and approved automobile insurance are required.
