Full-TimeHenderson, NC, USADescriptionThe Care Coordinator will provide evidence based person-centered care coordination services for at-risk Adult members with Mental Health or Substance Use Disorder (MHSUD) in a hospital setting. The Care Coordinator will work to proactively identify barriers to care, create targeted action plans for improvement, ensure quality and adherence to treatment plans, and work collaboratively with hospital. Contact with the hospital and community stakeholders is significant and the Care Coordinator is responsible for ensuring a high level of quality in customer service, including strong collaborative relationships and developing an interdisciplinary team approach to supportive treatment with providers and key stakeholders. This is a mobile position.Community Care Coordinators are responsible for establishing relationships with at-risk members and linking them to appropriate providers offering the necessary level of care. Role responsibilities include but are not limited to:
Utilizes a Whole Person treatment model to ensure that services for the individual are coordinated across the Cardinal Innovations service array and with other systems, including primary care
Receives member referrals from a variety of sources and assesses member needs based on clinical evaluation and best practice methods.
Completes necessary clinical assessments or screenings with members, face-to-face, to identify member goals and needs
Implements and manages assessments and care plans by linking to appropriate supports and monitoring engagement
Evaluates outcomes of service links for progression and completion of targeted goals
Convenes key providers and participates in an interdisciplinary team approach to address needs of the individual and ensure appropriate treatment planning
Identifies gaps in services and intervenes to ensure that the individual receives appropriate care including evaluation treatment plan goals for progression and completion
Adheres to defined tasks based on stratification criteria and ongoing monitoring efforts until member meets discharge criteria
Links appropriate members to population health management upon discharge from the crisis facility to have ongoing referral assistance and monitoring attendance in treatment post discharge
Completes all required clinical documentation or needs as well as identification of services for member linkage in a timely fashion as clinically indicated
Proactively ensures that individuals have a Behavioral Health Home and a Medical Home
Promotes person centered practices, including use of person centered language and person centered tools in planning
Focuses on achieving positive outcomes for individuals, including reduction in high risk events and inappropriate service utilization
Participates in clinical rounds/supervisor case consultations as indicated
Consistently adheres to established Cardinal protocols and timelines inclusive of documentation, quality guidelines, and established NCQA expectations.
This resource should be actively engaged in suggested and/or required training and education in an effort to continue the development and current knowledge necessary for the role.
This role will also maintain a working knowledge of the IT applications used for Care Coordination services.
In addition, the resource will remain compliant with quality guidelines and expectations set by the organization.
Supervision Received by EmployeeWork is completed with a considerable degree of independence. Team meetings and individual supervision will be provided with work reviewed by supervisor on a regular basis. Employee will be aligned with a geographically-appropriate local community office where supervision may be based. Employee is familiar with values and goals of Care Coordination and minimal daily instruction is required.Knowledge, Skills and Abilities:
Solid understanding of the organization's Care Coordination service offerings
Depth of clinical competencies related to behavioral health care coordination; physical health is a plus
Exceptional interpersonal skills and highly effective communication ability in both written and oral form
Ability to work effectively in team environment to accomplish company goals and to identify and resolve problems
Computer proficiency in e-mail, Microsoft suite applications (such as Word, Excel, Outlook, etc.), and the internet
Ability to demonstrate flexibility and adaptability to change and improvement of processes
Ability to assess personal strengths/needs and identify needed training to promote own professional development
Ability to exemplify a positive, solution-focused attitude and to promote same in others
High level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance
Ability to make prompt independent and clinically ethical decisions based upon relevant facts
Problem solving, negotiation and conflict resolution skills are essential to balance the needs of both internal and external customers
Understanding of the Diagnostic and Statistical Manual of Mental Disorders (current version)
Considerable knowledge of the Mental Health/Substance Use Disorder service array provided through the network of providers with additional knowledge in Medicaid B, C waivers and NC DMA Clinical Coverage policies. Intellectual/developmental disability experience and/or knowledge a plus
Must be detail oriented, able to organize multiple tasks and priorities, and to effectively manage projects from start to finish
Ability to communicate effectively in both oral and written format
Required Training and Education:A Master’s degree in a Human Services field (such as Psychology, Social Work, Counseling, etc) is required, along with at least two years of post-degree experience with the population served. Additionally, valid unrestricted licensure in profession (social work, counseling or psychology) is required.Working Conditions:This is a mobile position in which work will be completed in a variety of locations which include provider facilities/agencies, office setting, member homes, and the employee’s home. This position requires some travel and may require additional hours beyond standard workday. Cardinal Innovations reserves the right to call upon this employee during non-business hours as needed to meet the demands of the position. Employee must be able to alter daily operations to fit the demands of the position. Employee is required to have and maintain high speed internet access (with a minimum download speed of 10Mb/s) in home residence as a condition of employment.
