We are an adolescent level III female home.
We are hiring for multiple shifts.
Mon-Fri: PRN
Sat-Sun: 4:00 pm to 12:00 am and 12:00 am to 8:00 am and PRN
Purpose of Position: To provide quality care, treatment and education to the individuals we serve. To provide a safe and therapeutic home-style environment enhanced by comprehensive mentoring, partnerships and recreational opportunities.
Educational/Experience Qualifications:
High School Diploma, GED or equivalent
Experience in Human Service field, preferred
Dependable, Reliable and Trustworthy
Good Interpersonal Skills
Strong Understanding of Confidentiality and Client Rights
Excellent organizational Skills
Ability to follow verbal and written instructions
Ability to read and write
Duties:
Communicate relevant consumer information to supervisors, case manager and/or parents
Provide privacy and confidentiality of all consumer records
Assure consumers are free from abuse, mistreatment and/or neglect and report any incidents of such to supervisor
Treat each consumer with dignity and respect
Complement all documentation related to Treatment Plans
Develop and demonstrate a thorough understanding of philosophy and purpose
Develop familiarity with the Treatment Plan of each consumer served
Implement goals and recommendations and follow appropriate documentation procedures
Provide a positive atmosphere which facilitates growth and learning
Make decisions based on training and policies when unsupervised
Available for On-Call support
Other duties as assigned by management
Ensures the implementation of all agency policies and procedures by monitoring sires through observations, check sheets, and agency feedback
Ensures compliance with all required state and federal regulations
Maintains positive and proactive communication
Performs direct care
Ensures daily completion of reading logs, behavior points, journaling, goals, groups, and activities are completed
Documentation in service notes and contact notes
Check MARS, Service Notes, and Contact Notes addressing discrepancies, as needed
Be knowledgeable and up-to-date regarding any incidents; to include writing incident reports when necessary per state regulations and guidelines
Ensure the Medical Visit Form is completed for each consumer visit
Notify Management, as soon as possible, if a clients MAR and/or Count Sheet is inaccurate
Keep consumer quality of care a priority, checking health and safety and intervening before crisis occurs and alerting supervisor and CEO of any potential health or safety concerns.
Be knowledgeable and aware of the Federal Deficit Reduction Act and report any known occurrences of fraud, waste, and abuse to ED or posted OIG compliance reporting hotline
Maintain an accurate time sheet
Complete assigned projects in required timelines
Perform duties as requested by supervisor
