Job Summary: Under limited supervision, this person will market to perspective employers in order to obtain job placements for individuals with disabilities. Provide training and mentoring for new direct care staff on a daily basis in a community based environment. Essential Duties and Responsibilities: Other duties may be assigned as necessary. Must be available to work flexible hours including nights, weekends and overtime as required. Follow regulations and guidelines for supported employment according to the funder. Exercise judgment in implementing goals, policies and procedures. Marketing to perspective employers in order to obtain job placements for individuals with disabilities. Complete intakes regularly and ensure record keeping standards are maintained. Transport individuals to and from various locations in a safe vehicle while maintaining a confidential environment. Responsible for an average of 12 job placements per fiscal year. Must consistently meet quarterly revenue objectives. Participate in and provide input into individuals ISP (Individual Support Plan) and/or PCP (Person Centered Plan). Ensure that goals and plans are implemented. Submit accurate billing weekly by established deadlines. Maintain well organized record books that are in full compliance according to LIFESPAN state requirements. Assist in selecting potential individuals through screening and assessment procedures. Oversee and direct the activities of individuals while performing job duties. Facilitate onsite job instruction, ensuring that they are placed in the most appropriate and least restrictive job for their individual needs and interest. Work as liaison between the employer and employee to ensure needs are met. Establish and maintain good relationships with various individuals and agencies including area supervisors, referring agencies, VR counselors, parents/guardians, case workers, employers, etc. Submit accurate billing documentation, data sheets, detailed and concise progress notes, etc. within the established deadlines. Remain abreast of new trends by attending workshops, seminars, conferences, meetings, etc. as needed. Encourage the use of natural supports at the work site. Operate various types of equipment including copier, calculator, computer, smart phone, etc. Adhere to the principles of the LIFESPAN ChOICES Training Curriculum. Maintain individuals confidentiality at all times. Represent LIFESPAN in a positive manner. Abide by, enforce and participate in the implementation and ongoing oversight of all LIFESPAN safety standards and regulations. Supervisory Responsibilities: This position has no direct employee supervisory responsibilities. However, this position is responsible for supervising and coaching the individuals under his/her care. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience High school diploma or general education degree (GED) is required for this position. Two years of experience in working with individuals with disabilities or related experience is required. Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Reasoning Skills Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Physical Requirements Must be able to operate a variety of automated office machines and other equipment (e.g. computers, copier, wheel chairs, etc). Physical demands are in excess of sedentary work; position requires walking, standing, reaching, stooping, lifting, etc. Will lift no more than forty pounds independently. Must be able to lift and/or carry weights of up to one hundred and fifty pounds with assistance, to include the weight of the person being supported as needed. Other Skills and Requirements Must be certified in CPR/First Aid or able to obtain certification.
