Transitional Care Liaison
Location: Waynesville, OH 45068
Salary: $52,000 – $72,000
We are seeking a Transitional Care Liaison to manage long-term care admissions, coordinate referrals, handle reimbursement processes, and build strong relationships with residents, families, and referral sources. This role also includes supervising and developing departmental staff.
Qualifications:
Associate degree required; Bachelor’s preferred
Ohio RN or LPN license preferred
3+ years in long-term care, hospital, or clinical settings; 2+ years in admissions preferred
Healthcare sales/marketing experience a plus
Proficient in Microsoft Office; valid driver’s license
