Description of Duties:
Provides service coordination to individuals requiring community-based long-term residential care services.
Develops and maintains person-centered service plans for all individuals on assigned caseload.
Monitors the progress and effectiveness of service plan implementation.
Coordinates all needed services with health care and other providers.
Provides technical assistance, social and emotional support to consumers and families.
Prepares and submits all required case forms, reports, billing documentation and correspondence in an accurate and timely manner.
Preferred Qualifications:
4+ years experience in the developmental disabilities services industry
2+ years supervisory experience
Good verbal & written communication skills
Skilled in Microsoft Office products
Flexible Schedule
Positive attitude and good sense of humor
