Registered Nurse - Case Manager

It is the oldest and largest single hospice providerincluding one of the state's three in-patient hospice facilitiesin a 10,000 square mile region.

JOB SUMMARY
The Registered Nurse plans, coordinates, implements, evaluates home health and hospice services, and is experienced in nursing, with an emphasis on community health education. The professional nurse builds from the resources of the Organization & community to provide services to meet the needs of individuals and families within their homes. The Registered Nurse is responsible for cost efficient and effective patient care with quality outcomes.
POSITION QUALIFICATIONS
Educational requirements include graduate of an accredited school of nursing with current licensure to practice professional nursing in the State of Oregon. One to two years of recent medical/surgical care experience in an institutional setting. Ideal candidates will possess 1-2 years of home health/ hospice care experience. Valid Oregon drivers license and/or reliable transportation with current auto insurance as applicable.
SCHEDULE
Full-time (32- 40 hours week). Normal Shifts are Monday through Friday 8am - 5pm. You may be called upon to flex your hours (up or down) to meet the needs of the business. Additionally, you may be required to assist in providing on-call coverage 1-2 days a month to patients/families when necessary, and as assigned by the Supervisor.
BENEFITS

COMPENSATION: $32.00-46.78 DOE
