Licensed Practical Nurse
POSITION OVERVIEW
Guided by goodness, loyalty, faith, and fun; the LPN Wellness Coordinator manages the day-to-day healthcare operations of the Wellness Department including: maintaining standards in resident care services; ensures residents are treated with respect and dignity and ensures quality care as residents healthcare needs change.
ESSENTIAL JOB FUNCTIONS
Assists with supervision of the Wellness Department employees with full accountability for the performance
Demonstrates independent judgment and discretion. Solves problems using standard procedures. Knows when to refer issues to supervisor and when to handle them personally
Communicate and enforce policies and procedures including the Employers policies and procedures
Assists Wellness Director in timely and accurate completion of resident evaluations as required by state regulations and Compass Senior Living policy
Complete timely and objective incident reports and investigations
Creates, implements and maintains an accurate wellness record for each resident
Interact with the residents physician, pharmacist, and medical personnel as needed
Assists with the coordination of care to residents with third party providers such as therapy, home healthcare and hospice
Supervise the medication administration and/or assistance with self-administration of medication programs as required by state law. Supervise the storage, distribution and discontinuation of residents medication
Conducts periodic Medication Technician skills inventory checks, and periodic medication audits
Responsible for ensuring quality resident care and delivery of service throughout all shifts. May be required to work shifts in order to meet the residents needs and staffing requirements
Residents Rights
Maintain compliance and understanding of regulations regarding residents rights
Assists Wellness Director in providing staff education and re-education trainings as needed or identified
Promote and encourage residents to participate in meaningful and life enrichment programs and other activities
Follow, communicate and serve as a role model for company policies, procedures
QUALIFICATIONS AND EDUCATION REQUIREMENTS
Must have an LPN license in good standing with the state and a minimum of 1-2 years of relevant experience
Working knowledge of computers including Excel, Word, and MS Office is required
Working knowledge of a skill or discipline that requires basic analytic ability
Must be able to communicate effectively with residents, families, staff, vendors and the general public
Must meet all health requirements
Must be able to perform duties and responsibilities (Essential Job Functions) with or without reasonable accommodation
Must have valid driver license
Must pass criminal background check and nursing licensure validation as required by state law
Must have compassion for and desire to work with the senior population
