The RCM is required to have experience in Skilled Nursing Facility environment.

Must be proficient with PCC, MDS process and Care Plans.

Additional RN Care Manager responsibilities:

Assures completion of comprehensive nursing assessment of all residents upon admission and on an on-going basis until discharge. Documents resident contact in resident chart with name and title within 24 hours of admission. Implements and coordinates a plan of care for residents in pertinent areas. Assists with plan of care and interventions for effectiveness; assesses for accuracy of the MDS. Collaborates in making changes on the plan of care as needed. Completes the assigned portions of the MDS for Nursing. Signs the MDS form to signify completion of the process. Monitors medication and treatments are administered to assigned residents. Completes skin care rounds weekly. Practices according to state and federal regulations and facility procedure or other guidelines.

Safeguards residents' property. Maintains residents' self-esteem, privacy and confidentiality of personal information. Provides residents with opportunities for independence and choice consistent with their care plans. Keeps resident living areas pleasant and orderly.

Responds appropriately to residents' requests for help and maintains positive relationships with residents' friends and family members. Completes all documentation within established timelines. Receives and shares information; observes and reports residents' conditions to appropriate staff. Processes physician orders; follows facility systems of physician notification, communicates with Physician questions regarding medication, Makes rounds with Physician/Nurse Practioner/Physician Assistant as assigned, Reports residents' change of condition to staff and responsible parties timely.
